Position Summary:
This is a full-time opportunity for a Wound Care focused Nurse Practitioner who wants more from their career. In this role, you will operate with true clinical independence, managing complex wound cases while building long-term patient relationships. You will become a trusted clinical partner within your assigned communities, driving better outcomes and elevating the standard of care. Ideal for providers seeking professional growth, autonomy, and a break from traditional clinic or hospital settings.


Responsibilities:
  • Provide full-time, on-site rounding including primary care and advanced wound care services.
  • Manage complex wounds such as pressure injuries, diabetic ulcers, and post-acute cases.
  • Drive chronic care management and preventative care strategies.
  • Collaborate with physicians, nurses, therapists, and facility staff.
  • Reduce ER visits and hospitalizations through proactive interventions.
  • Communicate with families, caregivers, and interdisciplinary teams.
  • Complete documentation, medication management, and lab coordination.


Qualifications:
  • Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred).
  • Experience in wound care, geriatrics, primary care, or post-acute care preferred.
  • Valid driver’s license and ability to travel within the Twin Cities Metro.
  • Strong ability to work independently and manage a mobile schedule.
  • Passion for delivering high-quality, patient-centered senior care.
