Field Nurse Practitioner

Assist high-risk, high-cost Medicaid and AHCCCS members in their homes to improve health outcomes through care coordination, education, and medical management.

  • Conduct patient exams, review records, interpret tests, and develop care plans integrating medical practices.
  • Prescribe and reconcile medications, document encounters, and educate patients and families about medical needs.
  • Collaborate with healthcare providers and team members to ensure seamless care and address care gaps.
  • Manage approximately 100 members monthly, meeting with five in-home patients daily, focusing on goal achievement and transitional care.
  • Work closely with multidisciplinary teams, including medical assistants, social workers, and behavioral health specialists.

Qualifications include valid Nurse Practitioner license, experience in primary or acute care, excellent communication skills, and proficiency in patient engagement. Bilingual in Spanish preferred. Compensation ranges from $135,000 to $155,000 annually, with comprehensive benefits and performance bonuses. The role covers the Yavapai West area, with travel required for home visits.
