Forensic Case Manager - Manhattan Mental Health Court Evaluation Services

Salary: $27.81/ hour

Schedule: 9AM-5PM, Monday- Friday

Location: 175 Remsen Street in Brooklyn on Tuesday and Thursday. Courthouse at 100 Centre Street in Manhattan on Wednesdays and remote on Monday and Friday.

What drives the core purpose of this job:

The Forensic Case Manager provides services and identification, screening, treatment planning, case management, and court reporting of eligible participants living with mental illness or co-occurring disorders.

What are the key responsibilities:

  • Act as liaison to the courts and correctional facilities. Provide case-finding and screening services for cases within the Court in order to ascertain appropriate service plans for participants.
  • Follows and implements a comprehensive screening to effectuate referrals to treatment and accompanies and/or arranges transportation for clients to the appropriate facility when necessary.
  • Submit monitoring reports to the MMHC Resource Coordinator in a timely manner prior to each scheduled court appearance describing participants compliance with court approved treatment plans.
  • Immediately notify the court if participants commit major infractions.
  • Maintain case documentation available for review by supervision at any time, i.e. client records, including progress notes and all other relevant information to assist clients and to comply with program requirements.
  • Assist clients in applying for and receiving public and private benefits.
  • Maintain a caseload and monitor client participation in the treatment program through case management appointments, reviewing progress reports and conferencing cases with treatment personnel.
  • Make periodic visits to treatment facilities and maintain accurate log of visits.
  • Prepare and present written and oral status reports to the criminal justice system in a timely manner regarding client's progress.
  • Ensure weekly, bi-weekly, or clinically appropriate level of contact with program participants and treatment providers.
  • Participate in case conferences weekly or as needed.
  • Investigate and arrange supportive services and/or entitlements for clients.
  • Attend and engage in relevant trainings or continuing education opportunities as directed by Program Director.
  • Conduct supervised toxicology on participants as needed.
  • Act as a resource to staff members for guidance in treatment planning and treatment needs.
  • Assists Program Director with administrative tasks related to program management.

What key knowledge, skills, and abilities are required to excel in this role:

  • Bachelor's degree required in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology. Master's preferred
  • One or more years of experience working with the criminal justice, substance abusing, or mental ill population preferred.
  • Good communication skills, both oral and written. Ability to work cooperatively with treatment and criminal justice systems.
  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.
