Position Summary
The Case Manager supports case management services by helping coordinate care, resources, and services for clients. This role works closely with Case Manager/Supervisor to ensure clients receive timely, organized, and compassionate support while maintaining accurate documentation and communication.
Key Responsibilities
- Case Managers with client intake, assessments, and ongoing case coordination
- Help develop and track service plans based on client needs and goals
- Coordinate appointments, referrals, and follow-ups with foster parents and DHHS team
- Maintain accurate, up-to-date client records and documentation
- Monitor client progress and report changes or concerns to the Lead Case Manager
- Communicate with clients, foster families, and DHHS in a professional and supportive manner
- Ensure compliance with agency policies, confidentiality standards, and regulatory requirements
- Provide administrative support, including data entry, filing, and scheduling
Qualifications
- High school diploma or equivalent required; associate’s or bachelor’s degree in social work, human services, or a related field preferred
- Previous experience in case management, social services, healthcare, or a related field preferred
- Strong organizational and time-management skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a team
- Proficiency with basic computer systems and documentation tools
- Compassionate, detail-oriented, and client-focused approach
- Understanding of mental health
- Sucessfully complete and pass a background check
Work Environment
- May involve office, community, or field-based work depending on program needs
- Requires interaction with diverse populations and service providers

Pay: $22.00 - $23.50 per hour
