Foster Care Case Manager

Position Summary
The Case Manager supports case management services by helping coordinate care, resources, and services for clients. This role works closely with Case Manager/Supervisor to ensure clients receive timely, organized, and compassionate support while maintaining accurate documentation and communication.

Key Responsibilities

  • Case Managers with client intake, assessments, and ongoing case coordination
  • Help develop and track service plans based on client needs and goals
  • Coordinate appointments, referrals, and follow-ups with foster parents and DHHS team
  • Maintain accurate, up-to-date client records and documentation
  • Monitor client progress and report changes or concerns to the Lead Case Manager
  • Communicate with clients, foster families, and DHHS in a professional and supportive manner
  • Ensure compliance with agency policies, confidentiality standards, and regulatory requirements
  • Provide administrative support, including data entry, filing, and scheduling

Qualifications

  • High school diploma or equivalent required; associate’s or bachelor’s degree in social work, human services, or a related field preferred
  • Previous experience in case management, social services, healthcare, or a related field preferred
  • Strong organizational and time-management skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and as part of a team
  • Proficiency with basic computer systems and documentation tools
  • Compassionate, detail-oriented, and client-focused approach
  • Understanding of mental health
  • Sucessfully complete and pass a background check

Work Environment

  • May involve office, community, or field-based work depending on program needs
  • Requires interaction with diverse populations and service providers

Job Types: Full-time, Part-time, Contract, Internship

Pay: $22.00 - $23.50 per hour
