As a Home Health LPN at Trintiy Health At Home you’ll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

Minimum Qualifications

• Graduate of an accredited or state approved school of practical nursing
• Licensure asa LPN in the State of Michigan
• A minimum of one (1) years of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passedNational Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must possess interpersonal skillssufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
• Must havecurrent Driver’s license and reliable transportation to and from work site.
