Synergy Care is seeking a compassionate and motivated Physical Therapist to join our team at Concordia Life Plan Community SNF/ Concordia Senior Living AL/IL in Oklahoma City, Oklahoma. This is a full-time opportunity working in a supportive AL/IL community with an on-site outpatient clinic and SNF, allowing you to build meaningful relationships with residents while providing high-quality, patient-centered care.

What You’ll Do

• Provide high-quality physical therapy services to residents in an AL/IL community and outpatient setting/SNF

• Develop individualized treatment plans that promote mobility, independence, and quality of life

• Collaborate with an interdisciplinary healthcare team to deliver exceptional care

• Build meaningful relationships with residents and their families

What We’re Looking For

✔ Graduate of an accredited Physical Therapy program
✔ Licensed Physical Therapist or eligible for licensure in Oklahoma
✔ Passion for working with the geriatric population and improving functional outcomes
