Position Summary:

The Full-Time SOAR Case Manager is responsible for assessing, planning, coordinating, and monitoring services for older adults enrolled in the Supporting Older Adults with Resources (SOAR) program. This role ensures clients’ health, safety, and well-being by connecting them to appropriate services and supports while maintaining compliance with Maryland Department of Aging (MDOA) guidelines, agency policies, and HIPAA regulations. The SOAR Case Manager serves as a key liaison between clients, families, service vendors, and community partners.

Essential Duties and Responsibilities:

· Perform all duties in compliance with SOAR Program policies and applicable state regulations.

· Assess clients for service needs and income eligibility, including completion of annual financial applications.

· Develop, implement, and update individualized care plans based on client needs and available resources.

· Coordinate and communicate with service vendors to ensure timely and appropriate service delivery.

· Conduct required follow-ups, including:

  • In-home visits every six months to reassess needs and evaluate services.
  • General follow-ups every three months, or more frequently as needed.

· Refer clients to MAC, Inc. programs and community resources and follow up to ensure services are provided.

· Serve as a knowledgeable resource for community referrals related to aging services.

· Maintain accurate, complete, and secure client files in accordance with state and agency requirements.

· Participate in monthly supervisory meetings to review case management issues and concerns.

· Maintain strict compliance with HIPAA and confidentiality standards.

· Participate in trainings, workshops, and conferences as appropriate.

· Maintain a professional, respectful, and courteous demeanor when interacting with clients, families, and partners.

Qualifications:

Education & Experience:

  • Bachelor’s degree in Social Work, Nursing, Gerontology, or a related field preferred.
  • 3–5 years of experience working with older adults.
  • 2–5 years of experience in case management, care coordination, or a related field.

Knowledge, Skills & Abilities:

  • Strong knowledge of aging-related issues, community resources, and service programs for older adults.
  • Familiarity with SOAR Program requirements and eligibility standards (or ability to learn quickly).
  • Understanding of HIPAA and confidentiality requirements.
  • Strong organizational, communication, and documentation skills.
  • Ability to work independently and collaboratively in both office and field settings.

Compensation & Schedule

· Full-time, approximately 35 hours per week (Monday–Friday).

· No weekends.

· Hourly rate of $22, commensurate with experience.
