POSITION SUMMARY:

The GUIDE Care Navigator is responsible for coordinating care for patients enrolled in the CMS Guiding an Improved Dementia Experience (GUIDE) Model. This role manages a panel of dementia patients and caregivers, ensures compliance with CMS program requirements, and supports care coordination, quality reporting, and patient engagement.

As a Guide Care Navigator with our team, you will play a crucial role in enhancing the lives of our patients with dementia and their families through comprehensive care coordination.

QUALIFICATIONS:

  • Two (2) to three (3) years of healthcare experience required; case management, primary care, home health, hospice, or geriatrics preferred

  • Nurse, Social Worker, or Counselor preferred, but not required

  • Strong organizational skills with ability to prioritize in a fast-paced environment

  • Professional handling of confidential and sensitive information

  • Strong communication skills with interdisciplinary teams

  • Proficiency in Microsoft Office (Excel, Outlook, Teams) and general computer systems

ESSENTIAL JOB FUNCTIONS:

Patient Identification & Enrollment

  • Identify and outreach eligible patients for GUIDE program participation

  • Educate patients and caregivers on program benefits

  • Provide and document CMS-required beneficiary notifications

  • Facilitate enrollment and submit required forms to CMS

  • Manage patient consent and data-sharing preferences

Care Coordination

  • Manage an assigned panel of GUIDE beneficiaries

  • Conduct required monthly patient or caregiver contacts

  • Track and update patient acuity tiers and caregiver status

  • Communicate patient status to primary care providers and care teams

  • Support care planning and follow-up on identified needs

Program Compliance & Reporting

  • Maintain accurate patient rosters and census reporting

  • Track patient counts for PMPM/DCMP billing

  • Submit quality and program data to CMS dashboards

  • Monitor patient movement including enrollments, disenrollments, and tier changes

  • Maintain practitioner roster and service area tracking

Interdisciplinary Team (IDT)

  • Facilitate and coordinate IDT meetings

  • Collaborate with providers, social workers, and care teams

  • Ensure communication and execution of care plans

Training Requirements

  • Complete required dementia care training (didactic and experiential)

  • Pass required competency assessments

  • Complete ongoing annual training requirements per CMS guidelines

Other Duties

  • Perform other duties as assigned

PHYSICAL DEMANDS:

  • Frequent speaking, listening, and computer use

  • Visual acuity for documentation and data entry

  • Repetitive motion activities

  • Occasional bending, lifting, stooping, and walking

  • Possible local travel for meetings or training
