As the Harm Reduction Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:

Conduct outreach activities to engage the target population

Identify community partners to engage PWUD who are unaware of their status or untreated
-
Implement harm reduction strategies aimed at helping individuals minimize their immediate health and safety risks

Provide enrollment into the agencies Syringe Exchange Program

Facilitate brief assessments with the target populations
-
Through review of the assessment, identify community based services to provide client linkage
-
Facilitate linkage to services through escort as necessary
Harm Reduction Case Manager Minimum Salary: $23.08
Harm Reduction Case Manager Maximum Salary: $23.08
Harm Reduction Case Manager Schedule: Mon – Fri 8:30am – 4:30pm
Qualifications:

Associate's degree
-
1-3 years of relevant experience, Human Services
-
Personal Cellphone
-
Valid driver's license, proof of current car insurance and registration and access to a reliable, working vehicle
