Duties & Responsibilities


POSITION OVERVIEW: (Primary Purpose of Position)

Under the direction of the Associate Director for MDS, the MDS Head Nurse is responsible for coordinating, completing, and validating resident assessments in accordance with the Minimum Data Set (MDS) 3.0, the Resident Assessment Instrument (RAI) User's Manual, and applicable Centers for Medicare & Medicaid Services (CMS) requirements.

The MDS Head Nurse ensures that resident assessments are accurate, comprehensive, and completed within federally mandated timeframes to support resident-centered care planning, quality outcomes, and regulatory compliance. This role oversees the integrity of the RAI process, facilitates interdisciplinary collaboration, and ensures documentation supports Medicare and Medicaid reimbursement, quality reporting, and survey readiness.

The MDS Head Nurse works collaboratively with the Interdisciplinary Team (IDT), including physicians, nursing staff, rehabilitation services, social services, dietary services, and therapeutic recreation, to ensure accurate clinical assessments and the development of individualized care plans that meet residents' clinical, psychosocial, and functional needs.

ESSENTIAL FUNCTIONS: (List in order of importance to role and time spent on function)

Resident Assessment and RAI Process Oversight

  • Coordinates and oversees the Resident Assessment Instrument (RAI) process in accordance with CMS regulations and facility policy.
  • Ensures accurate and timely completion of MDS 3.0 assessments, including OBRA and Medicare PPS assessments, within required regulatory timeframes.
  • Manages appropriate scheduling of Assessment Reference Dates (ARDs).
  • Conducts comprehensive reviews of medical records, direct observations, and staff/resident interviews to ensure accurate MDS coding and documentation.
  • Validates that clinical documentation supports the coding of MDS items and reflects the resident's actual functional status and care needs.
  • Ensures all assessments comply with CMS, federal, and New York State Department of Health regulations. Attends and coordinate the interdisciplinary team comprehensive care planning meeting.

Care Planning and Interdisciplinary Coordination

  • Coordinates and facilitates Interdisciplinary Team (IDT) care plan meetings in accordance with CMS requirements.
  • Ensures the development of comprehensive, resident-centered care plans based on Care Area Assessments (CAAs).
  • Collaborates with nursing, rehabilitation, social services, dietary, and other disciplines to ensure they are updated before the care plan meeting.
  • Ensures that care plans are individualized, measurable, and reflect resident preferences, goals, and clinical needs.
  • Promotes effective communication among IDT members to ensure continuity and quality of care.

Regulatory Compliance and Survey Readiness

  • Maintains compliance with all CMS regulations governing resident assessments and care planning, including but not limited to:
    • 42 CFR §483.20 – Resident Assessment
    • 42 CFR §483.21 – Comprehensive Person-Centered Care Planning
  • Assists in preparation for state and federal surveys, audits, and quality reviews.
  • Participates in regulatory reviews and responds to inquiries related to MDS documentation and care planning processes.

Documentation, Quality Monitoring, and Data Integrity

  • Ensures that all MDS assessments are accurately coded, electronically submitted, and accepted by CMS systems.
  • Maintains and monitors MDS tracking systems to ensure timely completion and submission of OBRA and PPS assessments.
  • Reviews clinical documentation to ensure consistency with MDS coding and regulatory requirements.
  • Identifies trends, documentation discrepancies, and opportunities for improvement in the assessment process.
  • Participates in Quality Assurance and Performance Improvement (QAPI) initiatives related to resident MDS assessment.

Meetings and Interdisciplinary Collaboration

  • Attends

  • Communicates resident status updates and assessment findings to appropriate team members.

Professional Standards and Confidentiality

  • Maintains confidentiality of resident health information in accordance with HIPAA regulations and NYC Health and Hospitals policies.
  • Demonstrates professional accountability and adherence to organizational policies and clinical practice standards.
  • Performs additional duties as assigned to support facility operations and regulatory compliance.

Minimum Qualifications


1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and
3. One (1) year of experience as a registered nurse.

Department Preferences

  • Current New York State Registered Professional Nurse (RN) license and registration.
  • Bachelor of Science in Nursing (BSN)
  • Minimum clinical experience in a long-term care or skilled nursing facility setting.
  • RAC-CT (Resident Assessment Coordinator – Certified) credential.
  • At a minimum, must have knowledge of current state and federal regulations, particularly the following pertinent sections of Appendix PP:
  • §483.20 Resident Assessments
  • §483.21 Comprehensive Resident-Centered Care Plans
  • PRI (Patient Review Instrument) Certification.
  • Experience with PointClickCare (PCC) and PointRight analytics systems.
  • Knowledge of Medicare PPS reimbursement methodology and Medicaid case mix systems.
  • Strong understanding of CMS long-term care regulations and survey processes.
  • Comprehensive knowledge of MDS 3.0 coding guidelines and RAI Manual requirements.

Additional Salary Compensation


In addition to base compensation, you may also receive the following based on the Collectively-Bargained Agreement:

An annual education differential if you currently possess an appropriate degree in nursing or an allied health discipline from an accredited college or university, as listed below:

  • Bachelors in allied health discipline = $900
  • Masters in allied health discipline = $1,200
  • Bachelors of Science in Nursing (BSN) = $1,800
  • Masters of Science in Nursing (MSN) = $2,000
  • Doctorate in Education, Nursing Practice or PhD in Nursing (DNP) = $2,500

An annual certification differential ranging from $1,878 up to $4,878 if you currently hold appropriate certifications that are nationally recognized in the discipline or area of specialization for which you are employed.

An annual nursing experience differential ranging from $1,042 up to $8,645 for up to 10 years of experience if you have outside nursing experience as a Head Nurse.
