Health Homes Facilitator (Missouri)

We are seeking a dedicated and organized Health Homes Facilitator to join our team. This job will be hybrid (mostly remote but some on-site work could be required). A fixed weekly schedule will be required to ensure program compliance.

Job Overview: The Health Homes Facilitator plays a crucial role in the coordination and management of services for individuals within the Developmental Disabilities Health Homes program. This position ensures seamless operations by tracking enrollments, facilitating information exchange, and supporting both technical and general team needs, including assisting the nursing team with direct communications with consumers and/or guardians as needed. The facilitator will work closely with healthcare providers to monitor and improve the health outcomes of individuals enrolled in the program. High School Diploma required (or equivalent). Computer skills, time management, and organizational skills are a must.

Key Responsibilities:

· Enrollment and Discharge Management:

o Track individual enrollments, transfers, and discharges within the DD HH program.

o Maintain accurate records of all changes in individual status and report these to the necessary parties.

· Information Exchange Facilitation:

o Ensure efficient and effective communication between various stakeholders, including healthcare providers, individuals, and their families.

o Support the dissemination and receipt of necessary information to facilitate care coordination.

· Hospitalization and Emergency Tracking:

o Monitor and log individual hospitalizations, discharges, and emergency room visits.

o Identify high utilizers of healthcare services and coordinate with care teams to address their needs.

· Screening and Data Management:

o Track the completion of various health screenings, with a focus on Metabolic Screenings (MBS).

o Ensure accurate and timely entry of MBS data into the system.

· Technical Duties

o Assist with compiling, analyzing, and distributing data needed for program requirements (monthly reporting, Population Health, monthly attestations, etc.)

o Help support IT functions to ensure smooth operation of DD HH program systems.

o Assist team members with technical issues and facilitate training as needed.

· Team Support:

o Support the general operations of the DD HH team, contributing to a collaborative and efficient working environment.

o Assist in the coordination of team meetings and programmatic initiatives.

Qualifications:

· Must be at least 18 years of age.

· High School diploma or its equivalent is required.

Job Type: Part-time

Pay: $16.00 - $17.00 per hour
