HELM Case Manager-Duval/St. Johns

Provide targeted case management services to adults and children with severe and persistent mental illnesses enrolled in HELM program with both direct and indirect services to clients assigned to caseload.

Key Responsibilities

• Conduct initial and ongoing assessments of client needs, strengths, and goals
• Develop, implement, and regularly update individualized service plans
• Coordinate care with internal teams, healthcare providers, and community partners
• Link clients to resources including housing, employment, medical care, and social services
• Provide advocacy and support to ensure access to needed services
• Maintain regular contact with clients through in-person visits, phone calls, and community outreach
• Monitor client progress and adjust plans as needed
• Complete accurate and timely documentation in accordance with agency and regulatory standards
• Participate in team meetings, case reviews, and trainings
• Assist clients in crisis situations and follow appropriate intervention protocols

Bachelor’s degree in social work, psychology, human services, or related field (required)

• Relevant case management or behavioral health experience preferred
• Knowledge of community resources and social service systems
• Strong communication, organizational, and problem-solving skills
• Ability to work independently and as part of a multidisciplinary team
• Valid driver’s license and reliable transportation
• Successful completion of background screening

Job Type: Full-time

Pay: $19.00 - $20.50 per hour
