Responsibilities

• Provide attentive patient observation to monitor health status and respond promptly to changes.
• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support.
• Administer medications;
• Support patients with specialized needs, including hoyer lift.
• Operate assistive devices such as Hoyer lifts and perform catheter care related tasks with sensitivity and professionalism.
• Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring compliance with HIPAA regulations for patient privacy.
• Support home & community care initiatives by helping patients stay comfortable and engaged in their daily life.
• Work collaboratively within a team to deliver comprehensive home health services that meet each patient’s unique needs.

Qualifications

• Proven experience in home health care, assisted living, nursing homes, or long-term care environments.
• Familiarity with medication administration, and catheter care.
• Experience working with individuals with disabilities is highly valued.
