Position Overview:
The Senior Health Information Management (HIM) Deficiency Analyst performs daily analysis of the deficiency queues to provide guidance and leadership to the daily workflow with the HIM Deficiency Analysts . Manage the suspension queues to confirm and communicate with providers for timely completion of missing medical records and deficiencies. Performs deficiencies, reassigning deficiencies,
verification of the deficiencies with the clinical documentations, and manage the suspension processes.
Responsibilities:
Essential Functions
  • Initiates and performs technical procedures to analyze, or reanalyze, and monitor for medical record deficiencies according to Joint Commission and Medical Staff Rules and Regulations for regulatory requirements.
  • Perform the daily task to identify delinquent medical records according to the Joint Commission and Medical Staff Rules and Regulations for the regulatory requirements.
  • Execute the task of closing and verifying the episode of care by ensuring that all pertinent clinical documentations are in the medical record.
  • Manage the suspension process to ensure that all documents are complete and accurate in the episode of care to complete the chart
  • Perform the reanalysis and loose charts imaged separately from the discharged medical records.
  • Communicate with the Medical Staff Team and Chief to escalate the list of providers who are pending suspension.
  • Identifies providers with outstanding deficiencies, updates the providers’ statuses, and update the episode of care for the chart completion.
  • Assist and collaborate with providers with issues preventing them from completing the deficiencies and missing documents.
  • Escalate issues to the management team to obtain assistance with providers with deficiency and missing documentation oncerns.
  • Identify charts and analyze charts for concurrent analysis and assign deficiencies as required.
  • Train new HIM Deficiency Analysts on the systems and process to maintain the quality of the medical records.
  • Gathers statistical data regarding individual physicians’ record documentation and completion procedures for reporting to the Medical Records Committee and any other hospital committee as needed.
  • Performs audits as assigned to include: H & P, OB, Record Completion and others as assigned.
  • Meets productivity, timeliness, and quality standards and goals set by the department. Provides data from monitoring productivity and outcomes through maintenance of spreadsheets, database, and/or other documentation process.
  • Assists medical staff members in all aspects of record completion.
  • Understands and performs corporate suspension procedures for the individual facilities.
  • Processes physicians’ records that have been placed on leave of absence by Medical Staff Services.
  • Communicates cooperatively and constructively with physicians, patients, guests, and members of the healthcare team.
  • Performs all audits and compiles the data for reporting purposes.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.

Qualifications:
Education/Training
  • High school diploma or equivalent education.
  • Computer literacy and keyboarding skills required.
  • Must type a minimum of 35 words per minute.
  • Successfully passes a Windows proficiency test, scoring 75% or higher prior to an interview.
  • Medical terminology and anatomy and physiology coursework completion are required
Licensure/Certification
None.

Experience
  • Five (5) years of progressive work experience in Health Information Management field is required.
  • Completion of an approved HIM or HIT program may substitute one (1) year of experience.
  • Experience with chart completion and deficiency analysis.
  • Experience working with the hospital medical staff.
