Job Description

Join a dynamic team to provide a wide range of client-centered case management services to people living with HIV (PLWH) to expedite access to medical care with the goal of HIV viral suppression, to address medical, mental health, substance use, psychosocial, and other issues that are barriers to care, and to collaborate with other programs and with service providers so that clients can improve their health status. This position will serve a caseload of individuals impacted by the justice system.

Responsibilities include:

  • Provide services for people impacted by the justice system, including in person visits at local jails and home and community visits in order to ensure linkage to and retention in HIV care.
  • Coordination with justice center medical providers, public defender's offices and prosecutor's offices.
  • Comprehensive assessment to determine health and psychosocial needs.
  • Assessment of benefits, insurance, and other payer status and provision of assistance to access benefits programs.
  • Development of an individualized service/care plan to improve client’s health status and plan monitoring to assess progress towards goals.
  • Coordination of and referral to needed medical treatments or specialty care and follow-up to these.
  • Provision of assistance, advice, and/or referral to resources for supportive service needs.
  • Provision of treatment adherence counseling.
  • Provision of interventions needed to retain the client in care.
  • Provision of HIV, chronic disease and general health education to expand the client’s health literacy and improve general health.
  • Maintaining compliance with case management standards for clients assigned, including performing reviews and reassessments as required, updating service plans, and maintaining contact as required by client’s acuity level.

Qualifications

  • Required 2 year nursing degree OR bachelor’s degree in social work, public health or a related field. MPH or MSW preferred.
  • Personal or professional experience with HIV and the justice system.
  • 2+ years experience in case management or social work.
  • Must have private transportation and valid driver's license for home and community visits.
  • Proactive, reliable, team player with high quality communication and interpersonal skills.
  • Flexibility and adaptability, able to shift styles to fit the needs of a wide range of cultures, people, and organizations.
  • Excellent computer skills, including experience with Microsoft Office Suite.

Compensation and Benefits

  • Salary range of $46,000-$50,000 based on experience.
  • Full-time, exempt position, working 40 hours per week.
  • 12 sick days, 12-24 vacation days (based on length of time employed), 11 paid holidays, 12 Mental Health Days. Flexible, family-friendly schedule with main working hours Monday-Friday 9am-5pm. Occasionally outreach events will fall on an evening or weekend.
