Home-based Primary Care, Geriatrics and Palliative Care MD/NP/PA

We are currently seeking passionate and experienced Part-Time/Per-diem MD/PA/NP to join our innovative team delivering advanced geriatric primary care to older adults wherever they call home (House Calls).

Your Role:

- Field-based, making Primary Care House Calls (looking for clinicians in San Francisco, East Bay, and some Peninsula.

-Provide Longitudinal Geriatrics Primary Care

-Provide Palliative Care consultation when appropriate, and integrate palliative car into primary care when appropriate.

- Conduct comprehensive assessments, develop chronic care plans, and provide continuous care for older adults in their homes.

- Utilize your expertise in geriatrics, palliative care and primary care to make a meaningful difference in the lives of your patients.

- Fluent in Spanish is ideal.

Requirements:

• Valid California License
• Reliable transportation and current state driver's license and proof of auto insurance.
• Current DEA license.

Job Types: Full-time, Part-time

Pay: $82.00 - $95.00 per hour
