About the role
We're a multi-award winning, Joint Commission–accredited BrightStar Care franchise serving four NJ counties with both personal care and skilled nursing — which gives you something most home care reps don't have: a real clinical story to tell hospitals, case managers, and elder law attorneys. We have an established account base, a Director of Nursing who joins clinical visits, and an operations team that actually staffs what you sell.
We're looking for one outside rep to own Middlesex and Monmouth and grow it — with real room to expand the role as the territory scales.
What you'll do
- Build and maintain referral relationships with hospital discharge planners, SNF/rehab case managers, social workers, geriatric care managers, elder law attorneys, and senior living communities
- Run a weekly territory plan: in-person visits, lunch-and-learns, and quarterly events
- Partner with our Director of Nursing on joint clinical opportunities for higher-acuity accounts
- Track activity and pipeline; report weekly to the franchise owner
- Be the first call when a referral source has a client who needs care and the first to answer when a potential client reaches out
This role is roughly 35% office, 65% field. Office time is for planning, CRM, joint calls with the team, and coordinating with intake and scheduling. Field time is where the business gets won.

What we're looking for
- 2+ years outside sales experience; home care, hospice, DME, senior living, or pharma/medical device experience is strongly preferred
- Existing relationships with NJ discharge planners, case managers, or geriatric care managers is a major plus
- A competitive, assertive, results-oriented mindset with boundless energy
- Comfortable in clinical settings and confident talking with RNs and social workers
- Disciplined about activity tracking — we measure visits, not just closes
- A proven history of generating leads and then closing those deals
- Reliable car, valid NJ driver's license, clean driving record
