Home Care Aide

Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Support medication Reminder following care plans and DHHS regulations to ensure confidentiality
• Prepare nutritious meals tailored to individual dietary requirements and assist with feeding when necessary
• Conduct patient observation to identify changes in condition or behavior, especially in cases involving dementia, Alzheimer's, or other memory impairments
• Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces

Requirements

• Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
• Experience working with individuals with developmental disabilities, autism spectrum disorder, or dementia is a strong plus
• Ability to lift heavy objects safely and perform physically demanding tasks regularly
• Familiarity with care plans, medication Reminder, and patient monitoring protocols
• Strong communication skills for effective collaboration with healthcare teams and family members
• Commitment to maintaining HIPAA compliance and respecting client confidentiality
