Position Summary

The Home Health Aide (HHA) provides compassionate personal care and supportive services to patients in their homes under the supervision of a Registered Nurse or Licensed Practical Nurse. The HHA assists patients with activities of daily living to promote independence, dignity, and safety while following the individualized plan of care established by the clinical team.

Essential Duties and Responsibilities

  • Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.
  • Assist with mobility, transfers, and ambulation to maintain patient safety and comfort.
  • Provide assistance with personal hygiene and skin care.
  • Assist patients with meal preparation and feeding when necessary.
  • Perform light housekeeping tasks related to patient care such as laundry, dishwashing, and maintaining a safe living environment.
  • Observe and report changes in patient condition to the supervising nurse.
  • Take and record vital signs when required.
  • Provide companionship and emotional support to patients.
  • Follow infection control procedures and safety guidelines.
  • Document services provided and patient observations in the agency’s electronic documentation system.
  • Participate in care coordination and communicate effectively with the clinical team.

Required Qualifications

  • Current Home Health Aide (HHA) Certification required.
  • Minimum of 2–3 years of home health aide experience required.
  • Valid driver’s license with a clean driving record.
  • Reliable vehicle required.
  • Current automobile insurance required.
  • CPR certification required.
  • Current TB test or screening required.
  • Clear criminal background check required.
  • Ability to work independently and travel to patient homes.

Preferred Qualifications

  • Experience with Alora Home Health software preferred, but not required (training will be provided).
  • Experience working with elderly, disabled, or chronically ill patients.
  • Strong communication and patient care skills.

Physical Requirements

  • Ability to stand, walk, bend, and lift up to 25–35 pounds.
  • Ability to assist patients with transfers and mobility.
  • Ability to travel between patient homes throughout the service area.

Work Environment

Services are provided in patient homes and community settings. The Home Health Aide must be comfortable working independently in various home environments while maintaining professionalism and compassion.

Pay: $20.00 - $25.00 per hour
