Job Summary
Join our compassionate healthcare team as a Home Health Aide, where you will play a vital role in providing personalized, high-quality care to individuals in their homes. You will work closely with clients, families, and healthcare professionals to deliver comprehensive home health services that promote independence and well-being.
Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor and record vital signs including blood pressure, pulse, respiration, and temperature to track patient health status accurately
- Support medication administration following medical documentation protocols while maintaining HIPAA compliance for patient privacy
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques
- Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as directed by healthcare providers
- Utilize Electronic Health Record (EHR) systems to document patient interactions, care provided, and medical records thoroughly and accurately
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients
- Support patients during emergencies by recognizing symptoms requiring immediate attention and following proper protocols including CPR certification when applicable
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to observe patient conditions carefully and report changes promptly
- Familiarity with medical terminology and documentation practices within healthcare settings
- Experience working with individuals who have developmental disabilities or require specialized dementia and memory care is advantageous
- Knowledge of EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate record keeping
- Certification in CPR and first aid is required; additional certifications such as hospice care or long-term care experience are a plus
- Proven ability to handle caregiving responsibilities with compassion, patience, and professionalism in live-in or shift-based roles
