Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
  • Monitor and observe patients’ conditions, accurately documenting vital signs and changes in health status using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
  • Administer medications as prescribed, following all HIPAA regulations and medical documentation protocols to ensure patient safety.
  • Support clients with specialized care needs such as Alzheimer’s disease, dementia, hospice care, and memory care programs.
  • Operate medical equipment including Hoyer lifts, catheters, and other assistive devices to promote safe mobility and hygiene.
  • Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks like laundry and cleaning to maintain a safe living environment.
  • Collaborate with healthcare professionals by updating medical records and communicating patient progress or concerns effectively.

Skills

  • Proven experience in home health or long-term care environments with knowledge of medical terminology.
  • Strong patient observation skills to detect changes in health status promptly.
  • Experience working with individuals with developmental disabilities or cognitive impairments such as Alzheimer’s or dementia.
  • Familiarity with EMR/EHR systems for accurate documentation of patient care activities.
  • Ability to assist with complex procedures including catheter care, vital signs measurement, medication administration, and hospice support.
  • Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
  • Competence in providing compassionate senior care, home & community care services, and working within assisted living or nursing home settings.
  • Physical ability to lift patients using Hoyer lifts or similar equipment safely; flexibility to handle various caregiving tasks including cooking and cleaning. Pay: $21.00 - $24.00 per hour
