Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
• Monitor and observe patient conditions, including vital signs and overall health status
• Administer medication and perform basic medical procedures like catheter care and wound management under supervision
• Support patients with memory care needs, including Alzheimer’s and dementia care strategies
• Document patient interactions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with HIPAA regulations
• Prepare nutritious meals and assist with feeding, ensuring dietary restrictions are followed
• Provide light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment in accordance with care plans

Requirements

• Proven experience as a Home Health Aide (HHA), Nursing Assistant, or in assisted living or long-term care environments
• Knowledge of medical terminology, medication administration, and medical documentation practices
• Certification in CPR and first aid is required; additional certifications like hospice care or Hoyer lift training are a plus
• Ability to work independently and demonstrate excellent patient observation skills
• Experience working with people with developmental disabilities or memory impairments such as Alzheimer’s or dementia is highly desirable
• Familiarity with EMR/EHR systems for accurate record keeping and documentation
• Strong communication skills to collaborate effectively with patients, families, and healthcare teams
