Job Summary

Cypress Garden Center for Nursing & Rehabilitation is seeking a compassionate and dependable Home Health Aide (HHA) to join our team on a PRN basis. You will provide essential support to residents by assisting with transportation to medical appointments, ensuring safety during outings, and offering a caring presence throughout the day.

Key Responsibilities

• Escort residents to and from clinic appointments, dialysis treatments, and other off-site medical visits.

• Ensure resident safety during transportation and appointments, monitoring for fall risks or other concerns.

• Assist residents with preparation for appointments, including dressing and gathering necessary items.

• Provide snacks and support during out-on-pass periods as needed.

• Promptly report incidents, accidents, or changes in resident condition to the Clinical Coordinator or ADNS.

• Maintain professionalism and represent the facility positively while off-site.

• Adhere to all facility policies and procedures at all times.

Minimum Qualifications

• Current New York Home Health Aide (HHA) certification required.

• Strong communication and interpersonal skills.

• Dependable, flexible, and able to work independently.

• Compassionate and patient with a strong commitment to quality resident care.
