Job Overview
As a Home Health Aide, you will work under the supervision of a Registered Nurse or other healthcare professional to provide personal care and assistance with activities of daily living. You will play an important role in improving the quality of life for our patients and supporting their families.

Duties

• Assist patients with personal care, including bathing, grooming, dressing, and toileting.
• Assist with mobility, transfers, ambulation, and positioning.
• Observe and report changes in the patient's condition to the supervising nurse.
• Document care provided in accordance with agency policies and procedures.
• Follow the patient's individualized plan of care.
• Monitor and record vital signs such as blood pressure, pulse, and temperature to inform ongoing care.
• Follow HIPAA regulations diligently to protect patient confidentiality and privacy at all times.

Experience

• Current Home Health Aide certification (Required).
• Compassionate, patient-focused approach to care.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and a valid driver's license (Required).
• Ability to pass all required background clearances.
• Ability to lift, transfer, and assist patients safely.
