Job Summary

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of health status to develop personalized care plans.
  • Administer medications, including injections and IV infusions, following strict aseptic techniques and medication protocols.
  • Manage complex wound care procedures such as suturing, ostomy care, and dressing changes to promote healing and prevent infection.
  • Provide specialized treatments like Botulinum toxin injections for neurological or cosmetic purposes under physician supervision.
  • Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and adherence to hospice & palliative medicine standards.
  • Educate patients and family members on disease management, medication administration, nutrition, and safety measures within the home environment.
  • Document all patient interactions accurately using EMR systems like Epic or Cerner while maintaining HIPAA compliance and medical records integrity.

Skills

  • Extensive clinical experience in home health, hospice, or long-term care settings with a focus on patient-centered care.
  • Proficiency in electronic health record (EHR) systems such as Homecare homebase, Epic or Cerner for documentation and communication.
  • Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation techniques.
  • Skilled in performing complex procedures including IV insertions, phlebotomy, catheterization, tube feeding management, and sterile processing.
  • Critical care experience from ICU or emergency medicine environments enhances ability to manage acute pain and trauma cases effectively.
  • Ability to work with diverse populations including pediatrics, geriatrics, individuals with developmental disabilities or dementia care needs.
  • Familiarity with infection control protocols, laboratory testing procedures, and trauma medicine standards from Level I/II trauma centers or teaching hospitals is highly desirable.
