Job Description

We are seeking a licensed and compassionate Per Diem / PRN Home Health Physical Therapist to provide in-home physical therapy services to patients in Lehigh, Bucks, Montgomery, Monroe, and Northampton counties. The ideal candidate will be experienced in home health, patient-centered care, and skilled in developing individualized treatment plans to improve mobility, strength, balance, and overall functional independence.

This position offers flexible scheduling and paid mileage reimbursement.

Responsibilities

• Perform in-home physical therapy evaluations and treatments
• Develop and implement individualized treatment plans based on patient needs
• Assess patient progress and adjust treatment plans accordingly
• Educate patients and caregivers on exercises, safety, and fall prevention
• Complete accurate and timely documentation in the EMR
• Communicate effectively with physicians, nurses, and interdisciplinary team members
• Comply with Medicare regulations and agency policies

Requirements

• Current Physical Therapist license in the State of Pennsylvania
• Graduate of an accredited Physical Therapy program
• Minimum 1 year of experience (home health experience preferred)
• Knowledge of Medicare home health guidelines and documentation standards
• Valid driver’s license and reliable transportation
• Ability to work independently and manage time effectively

Schedule & Coverage Area

• Flexible PRN schedule
• Coverage needed in Lehigh, Northampton, Bucks, Monroe , and Montgomery counties
