Job Overview

We are seeking a full-time Home Health Physical Therapist to join our dynamic team. In this vital role, you will deliver expert physiotherapy services to diverse patient populations, including geriatric, pediatric, neurological, and orthopedic cases. Your expertise will help patients regain mobility, improve function, and achieve their health goals in a home setting. Responsibilities

• Conduct comprehensive assessments of patients’ physical conditions, including mobility, strength, and functional abilities.
• Develop and implement individualized care plans that align with physician orders and patient goals.
• Deliver evidence-based physiotherapy interventions such as trigger point therapy, orthopedics techniques, and gait training to facilitate recovery and improve function.
• Manage discharge planning processes by coordinating care transitions smoothly from inpatient or hospital settings to home or outpatient environments.
• Document patient progress accurately using electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
• Educate patients and caregivers on exercises, safety strategies, and health management techniques to promote ongoing wellness at home and in the community.
• Collaborate with healthcare professionals across inpatient, outpatient, clinic, and community settings to optimize patient care plans and outcomes.

Qualifications

• Valid state licensure or certification as a Physical Therapist with practicing in a physical therapy role.
• Proven experience in home health.
• Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, and geriatrics.
• Familiarity with Kinnser (preferred)
• Experience in discharge planning, treatment plan development, and working with geriatric or pediatric patients preferred.
• Excellent client communication skills with the ability to educate and motivate diverse patient populations.
• Ability to work independently in home environments while maintaining high standards of patient care.

Pay: $68.00 - $75.00 per hour
