Job Overview
We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to join our dynamic healthcare team. Responsibilities

  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
  • Administer medications, including injections and IV infusions, following strict aseptic techniques and safety protocols.
  • Manage complex wound care procedures such as suturing and ostomy care, ensuring optimal healing and infection prevention.
  • Provide education on disease management, medication adherence, nutrition, and self-care strategies tailored to each patient's needs.
  • Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and referrals for specialized services like hospice & palliative medicine or behavioral health.
  • Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate documentation of patient care activities and medical records.
  • Perform ongoing patient monitoring through telehealth triage or in-person visits, adjusting care plans as necessary to meet changing health conditions.

Skills

  • Proven experience in home health nursing with familiarity in medical-surgical, geriatrics, or long-term care settings.
  • Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques.
  • Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for efficient documentation.
  • Skilled in IV insertion, catheterization (including Foley and other urinary catheters), phlebotomy, and specimen collection/processing.
  • Experience managing acute pain and chronic disease conditions such as cardiac issues or infectious diseases.
  • Ability to perform physical examinations with attention to detail while demonstrating excellent patient assessment skills.

Job Type: Part-time

Pay: $35.00 - $125.00 per hour
