Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop personalized care plans.
- Administer medications, injections, IV infusions, and perform catheter care following strict aseptic techniques and medical documentation standards.
- Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while adhering to infection control protocols.
- Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient interactions, treatment plans, and progress notes.
- Provide education on disease management, medication administration, nutrition, mobility assistance, and health coaching tailored to individual needs.
- Coordinate discharge planning and collaborate with physicians and case managers to ensure seamless transitions of care across settings like hospitals, nursing homes, assisted living facilities, or outpatient clinics.
- Respond promptly to triage calls or urgent situations involving acute pain management, infectious disease care, or behavioral health concerns in the home environment.
Skills
- Proficiency with EMR/EHR systems such as Epic or Cerner for efficient documentation and communication.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to home healthcare.
- Experience with diverse clinical procedures including IV insertion, phlebotomy, tube feeding management, ventilator support, and laboratory specimen collection/processing.
- Familiarity with specialized areas such as hospice & palliative medicine, geriatrics—including memory care—and working with individuals with developmental disabilities or disabilities.
- Ability to perform comprehensive patient assessments in various settings like acute care hospitals (including Level I & II trauma centers), nursing homes, outpatient clinics or urgent care centers.
- Excellent critical thinking skills for triage decisions and emergency response in home settings; capable of managing complex cases involving trauma medicine or infectious diseases.
- Knowledge of Workers' Compensation law and HIPAA compliance to ensure legal and ethical standards are maintained at all times.
