Home Visiting Nurse Practitioner - Greater Houston Area, TX

We are currently looking for a qualified Licensed Nurse Practitioner: Internal Medicine Practice in the greater Houston area.

The incumbent of this position provides care in the patient’s home within the state’s scope of their practice. The nurse practitioner will primarily perform annual physicals and assist with chronic care management.

They will practice in an advanced role to provide advanced care for the geriatric home patients; including assessment, diagnosis, and treatment for their chronic diseases. Document patient care in accordance with regulatory, licensing, payer and accrediting requirements. Demonstrate the knowledge and skills necessary to consider developmental of the population served. Responsible for ensuring that all services are performed in a timely and cost effective manner. After evaluating patient's condition, you'll develop a specialized treatment plan. Ensures proper illness, injury care, evaluation, work-up, and recovery for patients. Relies on experience and judgment to plan and accomplish goals. May lead and direct the work of others.

This position is part-time (weekend flexible schedule). Compensation is per patient.

Qualifications:

• Graduate of an accredited nursing school with a Bachelor’s degree in nursing,
• Master’s Degree in nursing, national APRN Certification, and registered to practice as an APRN in the State of Texas.
• Must have current Texas RN license.

Job Types: Part-time, Per diem

Pay: $80.00 - $90.00 per hour
