WHAT YOU’LL DO

This role is the front door of hospice care—you set the tone for everything that follows.

  • Conduct hospice admissions in patient homes, ALF facilities, and SNF.
  • Perform comprehensive initial nursing assessments
  • Develop and initiate the Plan of Care in collaboration with the Nurse Practitioner and physician, and IDT.
  • Provide clear, compassionate education to patients and families about hospice services, expectations, and goals of care
  • Coordinate and initiate all services needed for admission, including:
  • Durable Medical Equipment (DME)
  • Medications (including comfort meds)
  • Supplies
  • Ensure a smooth and timely admission to services
  • Communicate effectively with referral sources, families, and the interdisciplinary team
  • Identify immediate needs and proactively problem-solve to ensure patient comfort and safety from day one
  • Complete all required documentation accurately and efficiently
  • Maintain compliance with hospice policies, procedures, and HIPAA regulations
  • Collaborate closely with case managers and the interdisciplinary group to ensure continuity of care
  • Participate in on-call/admissions coverage rotation as needed
  • Maintain clinical knowledge and skills

EXPERIENCE

  • ED, ICU, or high-acuity experience preferred
  • Admissions, triage, or rapid assessment experience preferred
  • Strong communication and organizational skills required

LICENSE/CERTIFICATION

  • RN License (Required)

WORK LOCATION

  • On the road (homes, facilities, hospitals)

Pay: $86,000.00 - $92,000.00 per year
