WHAT YOU’LL DO
This role is the front door of hospice care—you set the tone for everything that follows.
- Conduct hospice admissions in patient homes, ALF facilities, and SNF.
- Perform comprehensive initial nursing assessments
- Develop and initiate the Plan of Care in collaboration with the Nurse Practitioner and physician, and IDT.
- Provide clear, compassionate education to patients and families about hospice services, expectations, and goals of care
- Coordinate and initiate all services needed for admission, including:
- Durable Medical Equipment (DME)
- Medications (including comfort meds)
- Supplies
- Ensure a smooth and timely admission to services
- Communicate effectively with referral sources, families, and the interdisciplinary team
- Identify immediate needs and proactively problem-solve to ensure patient comfort and safety from day one
- Complete all required documentation accurately and efficiently
- Maintain compliance with hospice policies, procedures, and HIPAA regulations
- Collaborate closely with case managers and the interdisciplinary group to ensure continuity of care
- Participate in on-call/admissions coverage rotation as needed
- Maintain clinical knowledge and skills

EXPERIENCE

Pay: $86,000.00 - $92,000.00 per year
