Description

American Traveler is hiring a travel RN for a Hospice Admission Nurse position in Wisconsin, requiring 2 years of hospice experience and Epic EMR proficiency in a field-based home hospice role.

Details

  • Field-based role serving the home hospice program, with visits conducted in private homes, SNFs, and assisted living facilities
  • Primary territory covers the Sheboygan/Manitowoc area, with occasional travel to Green Bay or Oshkosh
  • Patient population consists of adults with diagnoses such as heart failure, cancer, and other end-of-life disease processes
  • Average caseload of 2–3 patients per shift, primarily completing hospice admissions or follow-up visits
  • Monday through Friday schedule; 8-hour or 12-hour shift options available (8:00 AM–4:30 PM or 8:00 AM–8:00 PM); rotating days off when working 12-hour shifts
  • No weekend rotation, on-call, or holiday requirements
  • EMR: Epic (required) — Epic Hyperspace and Epic Remote Client used for charting and secure chat
  • No floating requirement; travel between homes/facilities is typically no more than 45 minutes

Requirements

  • Active WI RN license or compact RN license currently in hand
  • Current BLS certification required
  • Valid driver's license, personal vehicle, and active auto insurance required
  • Minimum 2 years of hospice experience required
  • Epic EMR experience required
  • At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the past year
  • Candidate DOB and last 4 digits of SSN required for consideration
  • Candidate's middle name or initial required for consideration

Additional Information

  • Responsibilities include completing hospice admissions, conducting patient/family education on hospice care and end-of-life expectations, symptom management (pain, dyspnea), coordinating orders with physicians, and communicating with liaisons and case managers
  • May perform clinical interventions on initial visits as needed
  • Support available from a supervisor/manager via phone and chat, a liaison team, and hospice medical directors
  • Orientation includes general new employee orientation (NEO) and 40 hours of unit orientation with a staff member
  • Laptop and phone provided for use in the field
  • Dress code is business casual or professional scrubs
  • Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
