HOSPICE ASSOCIATE MEDICAL DIRECTOR
JOB DESCRIPTION:
The associate medical director is a remote position covering Wyoming Everwind Hospice locations (Sheridan, Gillette, and Casper). The Hospice Associate Medical Director provides clinical leadership and physician oversight for the hospice program, ensuring high-quality end-of-life care that meets regulatory requirements and aligns with the hospice philosophy of comfort-focused, patient-centered care. This role is structured as a per diem position, with compensation tied to average daily census (ADC) and weeks of active coverage.
Associate Medical Director shall be a physician licensed to practice medicine in the state of Wyoming. This position shall serve as acting Hospice Associate Medical Director and is intended to be a shared role with the Hospice Medical Director.
Provide medical direction for all hospice patients, including certification and recertification of terminal illness in accordance with CMS requirements
Review and approve plans of care in collaboration with the interdisciplinary group (IDG)
Ensure appropriate symptom management, including pain and complex symptom control
Be available for consultation to hospice staff regarding patient care needs
Actively participate in IDG meetings (approx. 1-2 hours per month)
Meetings occur every 14 days with the intention to rotate with the Medical Director
Provide clinical guidance to nursing, social work, and other team members
Assist in resolving complex clinical or ethical care situations
Ensure compliance with Centers for Medicare & Medicaid Services Conditions of Participation for hospice (42 CFR §418)
Complete and sign required documentation, including:
Certifications of terminal illness (CTIs)
Face-to-face encounter attestations
Support survey readiness and participate in surveys as needed
Collaborate with attending/primary care physicians to ensure continuity of care
Serve as attending physician when one is not designated or available
Provide education and outreach to community physicians as appropriate
Participate in the hospice Quality Assessment and Performance Improvement (QAPI) program
Assist in reviewing clinical outcomes and identifying opportunities for improvement
Provide physician coverage during assigned weeks including after-hours calls for urgent patient needs
MD or DO with an active, unrestricted license in the state of operation
Board certification in Hospice and Palliative Medicine preferred, but not required
Experience in hospice, palliative care, geriatrics, oncology, or internal medicine strongly preferred
DEA registration
Strong knowledge of hospice regulations and end-of-life care practices
Per diem model, based on time spent for IDG meetings plus Average Daily Census (ADC) tiers (0–15, 16–30, etc.)
Anticipated monthly time commitment (IDG prep and meeting, calls, etc.), less than 10 hrs/month
