Job Title: Hospice CNA
Location: Hybrid Remote Wellspring Hospice
Service Area: Saginaw, Bay and Genesee counties
Reports to: Clinical Manager
Position Type: Per Diem| Hourly

Wellspring Lutheran Services is a statewide nonprofit dedicated to serving seniors, families and children throughout Michigan. We are currently seeking a Hospice CNA to join our hospice team in Frankenmuth.

Responsibilities

• Provide personal care and services as outlined in the Plan of Care (POC), including bathing, grooming, dressing, feeding and mobility assistance.
• Observe, document and report changes in patient condition to the RN or appropriate skilled professional.
• Monitor vital signs (temperature, pulse, respiration, blood pressure) and recognize changes in patient status.
• Provide skin care to prevent breakdown and report concerns.
• Assist with toileting, incontinence care, catheter and colostomy care, enemas, and other tasks as competency allows.
• Support patients with exercises and range-of-motion activities as directed.
• Assist in medication administration as directed, following agency protocols.
• Maintain a clean, safe, and comfortable patient environment; perform light housekeeping and meal preparation as needed.
• Follow infection control procedures, universal precautions and organizational policies.
• Attend required trainings and educational sessions.
• Communicate effectively with patients, families, and team members, demonstrating professionalism and ethical standards.
• Respond to agency communications promptly for scheduling and updates.
• Recognize emergencies and initiate appropriate emergency procedures as needed.

Qualifications

• High school diploma or equivalent.
• Current & valid Michigan CNA license - required
• Valid Michigan driver’s license and reliable transportation.
• Preferred: 1 year of CNA experience in long-term care, home health, or hospice.
• Strong communication, observation, and organizational skills.
• Ability to work independently, manage multiple priorities, and demonstrate compassion and professionalism.

Schedule and Compensation

• Employment type: Pier Diem, 8 hours per week, Day Shift 8:00am-4:30pm
• Additional schedule: No weekends or holidays!
• Travel: Throughout the community to see patients in-home and in skilled nursing facilities - milage reimbursement provided
• Pay Rate: $18.00 per hour, based off experience and licensure.
