Hospice Director of Patient Care Services

Position Summary

This role is ideal for a seasoned Hospice RN or Clinical Manager who thrives in a collaborative, close-knit setting. You will lead our daily clinical operations while remaining willing to step into the field when needed to support our case managers, conduct supervisory visits, or assist with complex cases.

Key Responsibilities

  • Clinical Oversight: Direct and evaluate all clinical care operations, ensuring compliance with state regulations, Medicare Conditions of Participation, and TJC standards.
  • Team Leadership: Supervise, mentor, and evaluate clinical field staff (RNs, Social Workers, Chaplains, Aides) and lead weekly Interdisciplinary Group (IDG) meetings.
  • Quality & Compliance: Oversee QA/PI (Quality Assessment & Performance Improvement) initiatives, chart audits, and survey readiness.
  • Hands-on Support: Make occasional field visits for complex admissions, staff PTO coverage, or supervisory evaluations.
  • On-Call Backup: Participate in a balanced, secondary on-call rotation to support field nurses during after-hours emergencies or peak visit periods.

Qualifications & Requirements

  • Licensure: Active, unencumbered Registered Nurse (RN) license in Illinois.
  • Education: BSN required (MSN preferred).
  • Experience: Minimum 3 years of management experience in hospice or home health care.
  • Knowledge: Deep familiarity with TJC accreditation standards and Medicare Conditions of Participation.
  • Other: Valid driver’s license, reliable personal vehicle, and active auto insurance.

Work Location

  • Location: St. Charles, IL office with local field travel as needed.

Pay: $90,000.00 - $95,000.00 per year
