UPMC Family Hospice is hiring a full-time Home Care Aide. This position is a 100% field-based position and will be assigned to a specific Territory. Service area includes Lycoming, Union, Northumberland, Snyder, Sullivan, Clinton, Columbia, Montour counties, and parts of Centre County.
Must be able to work every 3-4 weeks and 2 holidays per year.
Purpose:
Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.
Responsibilities:
- Must maintain annual competency evaluation scores as determined by regulatory requirements

Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.

Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.

Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.

Participates in all required agency meetings.

Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to the appropriate professional staff.

Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.

Provides safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
