What You’ll Do:

• Deliver hands-on, compassionate care to patients in accordance with the State Nurse Practice Act

• Complete assessments and implement personalized care plans

• Document care and updates clearly and accurately

• Educate patients and families on diagnoses and care instructions

• Coordinate with the interdisciplinary team (Aides, PT/OT, MSW, Dietitian, etc.)

• Manage symptoms and pain effectively, while monitoring treatment responses

What You’ll Need:

• Graduate of an accredited LPN or vocational nursing program

• Active nursing license in the state of Ohio

• At least 1 year of medical/surgical experience (hospice or home health experience preferred)

• Current CPR certification

• Reliable transportation, valid driver’s license, and auto insurance
