Deliver hands-on, compassionate care to patients in accordance with the State Nurse Practice Act
Complete assessments and implement personalized care plans
Document care and updates clearly and accurately
Educate patients and families on diagnoses and care instructions
Coordinate with the interdisciplinary team (Aides, PT/OT, MSW, Dietitian, etc.)
Manage symptoms and pain effectively, while monitoring treatment responses
Graduate of an accredited LPN or vocational nursing program
Active nursing license in the state of Ohio
At least 1 year of medical/surgical experience (hospice or home health experience preferred)
Current CPR certification
Reliable transportation, valid driver’s license, and auto insurance
