The LVN Team Assistant position is a Full-Time position in our Encino location and surrounding areas.

Schedule: Day shift, 7:30am–4:30pm, with rotating every-other-weekend coverage.

Requirements:

• Current LVN license in the State of California
• Minimum of one year acute-care experience
• Proficiency with EMR systems and general computer applications
• Hospice experience preferred

Key Responsibilities:

• Assess daily staffing patterns and schedule patient visits to ensure continuity of care and equitable staff assignments
• Collaborate with acute hospitals, skilled nursing facilities, and community medical groups to plan and coordinate hospice services
• Arrange needed supplies, equipment, and other patient care resources
• Maintain positive relationships with contracted facilities, hospitals, referring physicians, managed care organizations, and payor partners
• Provide excellent customer service and route specific inquiries to the appropriate team members
• Support the Hospice Director by assigning patients to nurses for program explanations and/or timely admissions
• Communicate all referrals, nurse assignments, and admission time frames to the Hospice Manager
