Job Summary
Join our dedicated hospice care team as a Hospice On Call PRN RN PT/FT, where your expertise will provide compassionate, high-quality nursing care to patients in their homes and community settings. Duties
- Provide on-call nursing assessments and interventions for hospice patients in response to urgent needs or emergencies.
- Conduct comprehensive patient evaluations, including vital signs, symptom management, and medication administration, ensuring adherence to hospice protocols.
- Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to each patient's evolving needs.
- Manage complex symptom control such as acute pain management, dyspnea, nausea, and other distressing symptoms with appropriate medications and interventions.
- Document all patient interactions accurately within electronic health record (EHR) systems like Epic or Cerner, maintaining compliance with HIPAA regulations.
- Educate patients and families on disease progression, medication use (including injections), and comfort measures to promote understanding and peace of mind.
- Respond swiftly to after-hours calls, triaging urgent issues such as respiratory distress or sudden changes in condition, coordinating with physicians as needed.
Qualifications
- Current RN licensure with active state registration; prior hospice or palliative care experience preferred.
- Strong clinical background including critical care experience, ICU or emergency medicine background highly valued.
- Familiarity with medical documentation systems such as Epic, Cerner, or eClinicalWorks; proficiency in EMR/EHR systems is essential.
- Knowledge of pain management techniques, medication administration (including IV infusion), aseptic technique, and catheter care.
- Experience working with diverse patient populations including geriatrics, behavioral health cases, memory care residents, and individuals with developmental disabilities.
- Ability to perform physical assessments, vital signs measurement, specimen collection/processing, and basic life support (BLS).
- Excellent communication skills for effective triage and patient education; compassionate approach to end-of-life care is essential.
