FULL TIME Hospice RN/Case Manager

Starting yearly Wage Range - $75,000.00 to $80,000.00 (Based on Exp. & Patient Census)

Weekend On-Call Flat Rate $150.00

Mileage reimbursement

PTO (Will begin to accrue per pay period after 90 days)

-Previous RN Hospice Case Manager Experience a MUST

-Patient Admission Experience a MUST

-Strong Oral Communication

-Well Developed Interpersonal Skills

-Attentiveness to Details

-Experience with Medicare/DHS and Organizational Accredidation

-Supervisory Skills

-Timely documentation completion

-Confident with little need for Supervision

-Flexibility due to Patient needs and Census fluctuation

We are a small hospice organization that services the West/East valley areas. We are a strong team of professionals that enjoy working with Hospice patients and their families during end of life scenarios. Seeking an experienced RN for Hospice Case Management, admissions and on-call visits. Must be flexible and available for needs of patients. You will be responsible for case management, interfacing with core hospice staff, supervising hospice aides, patient record keeping on EHR (Kinnser) and attending mandatory bi-weekly IDG meetings. Must have a registered nursing degree, valid auto insurance, proof of current TB test and COVID vacination record if available.

Pay: $75,000.00 - $80,000.00 per year
