Our Hospice in Fairfax has an immediate openings for an on call PRN nurse working Saturdays and Sundays from 8am-5pm providing patients and their families with exceptional end-of-life care that supports dignity and comfort for individuals, peace of mind to families, and honors cultural traditions and customs in the Fairfax, Loudoun, Prince William counties.
Functions:- The nurse will provide information and admission visits as needed and deliver quality hospice and palliative care services to patients in their place of residence
- Will perform initial assessment of patient’s needs, serve as a clinical resource for referral sources and patients/family members, facilitate end of life conversations and decision-making with the family or caregiver.
- Will represent our organization when communicating with patients, families, and physicians.
Qualifications:- An active State of Virginia RN license and a minimum of 3 years of acute care and/or hospice experience.
- Demonstrated knowledge and skills necessary to provide meaningful care and to communicate with the terminally ill.
- Excellent communication and customer service skills; attention to detail; strong organization skills, and proficient computer skills.
- Proficiency with MS Office 365 and EMR systems. Familiarity with the Homecare Homebase electronic medical record a plus.
- A valid driver’s license and reliable, independent transportation to provide care to hospice patients in Northern Virginia.
- Must be available from 8am-5pm on call every Saturday and Sunday
