The Social Worker position is a Full-Time position during our day shift: 8:00am–4:30pm. Rotating on call after hours and weekends.

Requirements:

• Master of Social Work (MSW) degree
• Minimum of one year of direct patient care experience in a community or home care setting
• Hospice experience preferred; EMR experience preferred

Key Responsibilities:

• Participate in developing and updating each patient’s plan of care upon admission and when conditions change
• Ensure quality and safe delivery of hospice services
• Connect patients and families with appropriate community resources to address immediate needs
• Provide information about support services available to patients and families
• Assist families with mortuary arrangements as needed
• Conduct psychosocial assessments addressing emotional, social, and coping factors related to illness, prognosis acceptance, and the dying process
