Hospice Social Worker

Overview
Join our compassionate hospice team as a Hospice Social Worker, where you will play a vital role in delivering holistic, patient-centered care to individuals facing life-limiting illnesses. In this dynamic position, you will provide emotional support, psychosocial support , and case management services to patients and their families, ensuring comfort, dignity, and quality of life during challenging times. Your expertise will help navigate complex emotional and social needs while collaborating with interdisciplinary teams to deliver comprehensive hospice care. Duties

  • Conduct thorough patient assessments to evaluate social, emotional, and psychological needs within the hospice setting
  • Offer support, education, and coping assistance to patients and families based on individual needs and diagnoses
  • Attend monthly grief group support offered by Honeybee Hospice to the community
  • Develop personalized care plans incorporating social work principles, medical documentation, and HIPAA compliance to ensure privacy and effective communication
  • Collaborate with medical teams to coordinate hospice services, including intake procedures, discharge planning, and post-acute care strategies
  • Assist with data collection for clinical research or quality improvement initiatives related to hospice care outcomes

Requirements

  • Bachelor's or Master’s degree in Social Work (MSW preferred) but willing to train the right candidate
  • Proven experience in hospice care, inpatient settings, nursing homes, assisted living facilities, or home & community care environments
  • Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management
  • Familiarity with EMR systems for documentation and data collection purposes
  • Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disorders such as autism or Alzheimer's disease
  • Expertise in behavioral health therapies like cognitive behavioral therapy (CBT), applied behavior analysis (ABA), trauma-informed care (PTSD), and crisis management techniques
  • Excellent communication skills with the ability to conduct diagnostic evaluations and case management effectively
  • Ability to work independently while collaborating within multidisciplinary teams in outpatient or outpatient clinic settings

Pay: $25.00 - $27.00 per hour
