Hospice Social Worker

Job Overview
We are seeking a compassionate and dedicated Hospice Social Worker to join our team and provide vital emotional, psychological, and social support to patients and their families during one of life's most challenging times. In this role, you will collaborate with interdisciplinary teams to develop personalized care plans, assess patient needs, and facilitate a comfortable and meaningful end-of-life experience. Your expertise in behavioral health, mental health counseling, and social work will help ensure patients receive comprehensive care that respects their dignity and wishes. Duties

  • Conduct thorough patient assessments to identify emotional, social, and psychological needs related to hospice care.
  • Develop and implement individualized care plans incorporating clinical counseling, group therapy, and behavioral health strategies.
  • Provide individual and group psychotherapy sessions, utilizing techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and crisis intervention.
  • Offer family counseling and support, including child & family counseling when appropriate, to help loved ones cope with grief and caregiving challenges.
  • Collaborate with medical teams to ensure accurate documentation of medical records, HIPAA compliance, and medical terminology usage within electronic medical record (EMR) systems.
  • Facilitate intake processes for new patients, ensuring comprehensive case management and coordination of services across home & community care settings.
  • Assist with behavioral health interventions for patients with complex conditions such as dementia, Alzheimer's disease, PTSD, or developmental disabilities.
  • Participate in clinical research or quality improvement initiatives related to hospice care practices.
  • Maintain confidentiality and adhere to all regulations regarding medical documentation and HIPAA standards.

Experience

  • Proven experience in hospice care or palliative settings with a strong understanding of end-of-life issues.
  • Background in behavioral health counseling including mental health counseling, addiction counseling, or psychotherapy.
  • Experience working with diverse populations such as seniors in assisted living or nursing homes, children with special needs, or individuals with disabilities.
  • Familiarity with inpatient, outpatient, or home & community-based healthcare environments.
  • Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding for billing purposes, and medical records management.
  • Skills in crisis management, diagnostic evaluation, case management, and working effectively within EMR systems.
  • Ability to conduct assessments involving diagnostic tools for dementia care or autism spectrum disorders when applicable.
  • Strong communication skills for engaging patients through motivational interviewing techniques and providing compassionate psychosocial support during difficult times.
