  • Review and interpret physician prescriptions/medication orders for accuracy, appropriateness of dose, drug interactions, allergies, and patient-specific considerations.
  • Dispense medicines (tablets, capsules, injections, syrups, IV fluids, other formulations) to in-patients and out-patients, ensuring correct dosage, proper labeling, and safe administration.
  • Maintain and manage pharmacy inventory: ensure stock availability, proper storage (including temperature-sensitive medicines), check expiry dates, monitor stock levels; reorder medicines as needed.
  • Conduct medication reconciliation when patients are admitted or discharged: review their current medication history, reconcile previous and new medications, prevent duplication or harmful interactions.
  • Provide drug-information and counselling to patients (or their guardians / parents), including instructions on dosage, administration schedule, potential side-effects, drug interactions, and general medication safety.
  • Collaborate with doctors, nurses, and other health-care professionals to develop individualized medication plans and ensure optimal therapeutic outcomes.
  • Monitor patient responses to medications; observe and report any adverse drug reactions (ADRs); suggest changes or interventions as needed.
  • Maintain accurate records: prescriptions, patient medication profiles, stock/inventory logs, expiry and batch data, drug utilization, dispensation records, potentially controlled substances if applicable.
  • Ensure compliance with legal and regulatory requirements related to drug storage, labelling, dispensing (under applicable laws/regulations, e.g. the local/state pharmacy regulations or national drug regulations).
  • Maintain hygiene, cleanliness, and safety standards in pharmacy operations.
  • (If serving pediatric patients) Apply pediatric-specific knowledge: weight-based dosing, pediatric pharmacokinetics/pharmacodynamics, safe preparation of pediatric formulations, and counselling guardians/parents.
