Host Home Provider

Position Overview

Blessings in Disguise Inc. is seeking a compassionate and reliable Host Home Provider (HHP) to partner with our agency under the PASA IRSS program. As a Host Home Provider, you will open your home to an individual with developmental and/or intellectual disabilities, offering them a safe, stable, and family-like environment. This role is an opportunity to provide care that promotes dignity, independence, and inclusion in the community.

Responsibilities

• Residential Support – Provide safe, stable housing in your home with access to meals, hygiene facilities, and personal living space.
• Daily Living Assistance – Support with ADLs and IADLs (bathing, grooming, dressing, meals, laundry, housekeeping).
• Health & Medical Support – Administer or supervise medications as directed, monitor health conditions, and ensure access to healthcare.
• Behavioral Support – Provide continuous supervision, redirection, and guidance for behavioral needs as outlined in the individual’s service plan.
• Community Integration – Encourage and support participation in social, recreational, and community-based activities.
• Safety Oversight – Maintain a safe environment, monitor for risks, and follow emergency procedures when needed.
• Documentation & Compliance – Maintain accurate daily notes, incident reports, and communication logs in accordance with HCBS, IRSS, and CDPHE standards.
• Empowerment – Support individual choice and independence to the greatest extent possible while respecting cultural values and personal preferences.

Qualifications

• Must be at least 21 years old.
• Ability to pass all required background checks (criminal, CAPS, DMV).
• Willingness to complete all required PASA/IRSS trainings (medication administration, abuse/neglect prevention, first aid/CPR, behavior support).
• Stable home environment that meets health and safety standards.
• Compassionate, patient, and committed to supporting individuals with developmental disabilities.
• Strong interpersonal and communication skills to work collaboratively with families, case managers, and Blessings in Disguise staff.

Compensation

• Host Home Providers are independent contractors paid a daily rate (per diem) determined by the individual’s level of need and state funding authorization.
• Rates are designed to cover housing, meals, utilities, transportation, and caregiving services provided in the host home setting.
