Housing Case Manager

Overview
Watertown Urban Mission, (WUM), is seeking a dedicated and experienced Housing Case Manager for its grant funded Housing Programs. The ideal candidate will possess a strong background in human services case management with knowledge of poverty, substance abuse, behavioral health and homelessness, as well as a thorough understanding of Fair Housing regulations and habitable housing. This role requires excellent negotiation skills, advocacy and the ability to manage crises effectively.

Responsibilities
- Manage a caseload of clients experiencing homlessness and assist in finding stable housing.
- Provide intensive case management services to ensure stable housing is maintained.
- Advocate on client's behalf to assist in maintaining stability while offering resources and referrals to achieve their goals.

-Maintenance of accurate client charts.
-Knowledge of Fair Housing laws as well as standards of habitability.

-Ability to conduct Street Outreach.

Qualifications
- Have a thorough understanding of program grant contracts, to ensure eligibilty.

- Experience in providing intensive case management services to individuals experiencing homelessness.

-Ability to serve as a liaison between the client, landlord, other Mission programs, local Department of Social Services, specialized supportive services (ex., domestic violence, mental health, and/or substance abuse services providers), and any other service providers to ensure the client is receiving all appropriate benefits and services (ex., financial assistance, money management services, etc.) and meets eligibility of Housing programs.

-Maintain complete and accurate documentation for all clients, including case files with submitted documentation, case notes detailing all contacts with clients and all others (landlords, other service providers, etc.) who are involved in the clients’ journey to permanent housing .

- Knowledge of housing habitability standards.
