Overview
Join our dedicated affordable housing NFP team as a Housing Case Manager and become a vital advocate for vulnerable individuals experiencing housing instability and homelessness. In this dynamic role, you will empower tenants by not only providing housing but providing comprehensive support and connecting them to vital resources to successfully maintain their housing.
Responsibilities
- Provide supportive housing case management for tenant case load within program regulations, policies, and procedures. Ensure that all tenants receive quality services from agency and coordinated outside agencies.
- Provides information, referrals, crisis intervention, direct service, and advocacy services to all tenants.
- Ability to work both independently and as part of a team, be organized, assume responsibility for assigned work, detailed oriented and ability to be flexible.
- Participate in weekly supervision, team and staff meetings.
- Have reliable transportation to travel to see tenants within Northern Westchester. Mileage reimbursement available.
- Have strong problem-solving skills. Conduct monthly tenant meetings.
- Screens applicants referred to program and facilitate admissions with the admissions committee. Process applicant inquires.
- Assist tenants with direct services such as budgeting, entitlements, problem solving and conflict resolution as needed.
- Responsible for face-to-face meetings with tenants regularly. On call for emergencies only, 24/7.
- Completes all data entry into software programs in a timely manner. Ensure tenant documentation is complete and up to date including progress notes and service plans.
REQUIREMENTS
- Education: Bachelor’s degree in human services. Extensive experience may be substituted for educational requirements.
- Experience: Two years’ experience, working in a related field such as mental health, homelessness, healthcare or housing.
- Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus.
- Computer skills: Basic competency in MS Word, Excel, Outlook and ability to learn program software.
- Physical Performance: Ability to make home visits (walk distances, climb stairs, etc.).
- Driving Record: must maintain a good driving record as a condition of employment and continued employment
- Background Check; Must pass a CBC background check as a condition of employment
Pay: $44,850.00 - $48,750.00 per year
