Housing Case Manager

Our Case Management Services help individuals create fulfilling lives with maximum independence by providing advocacy, support, and linkage to community resources.

Job Scope: Deliver home and community-based case management services to clients referred under the Housing is Recovery program. Support clients in securing and maintaining housing, increasing independent living skills, accessing entitlements/benefits, providing wrap-around support, and linkage to mental health or substance use services as appropriate. Build relationships with referral sources such as hospitals, county jails, prisons, and homeless shelters. Identify and navigate affordable housing opportunities through relationship-building with landlords and property management companies. Collaborate effectively with these community partners to address any reported behavioral concerns and proactively manage potential barriers with the participants ability to maintain their housing and independence in the community.

Primary Responsibilities

• Provide home- and community-based treatment to and assists in developing case management services to a caseload of clients who are referred for the Housing is Recovery Program. Services assist clients in maintaining housing, providing wrap-around support, including linkage to mental health or substance use recovery support services.
• Build relationships with referral sources, including hospitals, county jails, prisons, homeless shelters, and inpatient detoxification facilities.
• Ensures Housing Quality Standard (HQS) inspections on all apartment units are conducted and communicates results to landlord.
• Complete Mental Health Assessments, DLA and LOCUS, and develop goal-oriented ITPs that are strength-based and recovery oriented on all assigned clients as required.
• Accompany clients on housing searches and support all needs surrounding securing housing for the clients required by the Housing is Recovery Program.
• Supports program activities associated with being a subsidy administrator.
• Provide assistance with intake/assessment as needed, as well as outreach in emergency situations
• Provide accurate, quality documentation within a maximum of 48 hours after service delivery

Performance Requirements

• Ability to assess, diagnose, interpret and integrate both verbal and non-verbal client input during session.
• Ability to clinically “listen” during session.
• Establishing rapport and maintaining supportive contact with client.
• Knowledge of human development across the life cycle as applicable to the clinical population served.
• Courteous, effective relationships in the workplace.
• Knowledge of agency policy and adherence to agency procedure.
• Ability to value diversity, accept differences, apply cultural knowledge, and adapt to different cultural contexts.

Education and Experience

• Bachelor’s degree in human service field and minimum 1year clinical experience, including case management work and work with individuals that are diagnosed with severe mental illness and/or co-occurring substance use needs.
• Bilingual Spanish a plus
• Must have valid Illinois driver's license, auto insurance and vehicle and willingness to transport clients

Schedule

• Monday – Friday 9:00am – 5:30pm (40 hours a week).

Pay: $52,520.00 - $59,176.00 per year
