Role Overview:
Responsible for the delivery of case management services to the residents of the Phoenix Rising program. Work in collaboration with the treatment team, housing team and community providers to provide systems coordination, develop a Housing Stability Plan with the residents, and assist the residents towards successful attainment of their Housing Stability Plan goals. The Case Manager will assess residents’ strengths and obstacles to stable housing. The Case Manager will assist the resident through the intake/move-in process and when exiting to permanent housing. The Case Manager will facilitate regular community meetings and work with residents daily to develop and maintain a safe and healthy community. The Case Manager will develop and facilitate in-program groups in a variety of subjects important to assisting residents to move into independent, permanent housing and stabilize recovery.
Key Responsibilities:
  • Conduct a housing needs assessment with clients to identify barriers to housing.
  • Assist clients in developing an individualized Housing Stability plan addressing barriers to housing.
  • Assist clients in completing housing applications, including KCHA housing application packets.
  • Collaborate regarding the clinical and housing needs of program clients.
  • Assist clients with budgeting and related planning processes in preparation for housing applications and search. Advocate on behalf of clients and their rights.
  • Assist clients with obtaining necessary documents such as credit reports, background check, etc.
  • Educate clients about their rights and responsibilities as a tenant.
  • Assist clients in developing appropriate communication with landlords & program staff.
What We're Looking For:
Education:
  • BA degree in human services.
Desired Experience:
  • Prior experience with homeless Young Adults, individuals or families.
  • Knowledge of landlord tenant law, housing authority regulations, the provisions of a standard lease and related information.
  • Knowledge and skills in providing clinical case management.
  • Demonstrated ability to work effectively with a variety of professionals and a diverse client population.
Other:
  • Current Driver’s License.
  • Current Automotive insurance: If transporting clients in personal vehicle.
  • Washington State Counselor Agency Affiliated Registration.
  • Pass required background checks.
  • Ability to transport clients.
  • Ability to work independently.
  • Ability to work evenings.
